Five Categories For diabetes, Not Just Type 1 and Type 2


The diabetes was classified into two types- Type I and Type II. Not really!

According to a research published in The Lancet Diabetes & Endocrinology, a leading medical journal, there are five distinct types of diabetes that can occur in adulthood.
Currently, the disease is divided into two sub-types.

  • With Type 1 — generally diagnosed in childhood and accounting for about 10 percent of cases — the body simply doesn't make insulin, a hormone that helps regulate blood sugar levels.
  • For Type 2, the body makes some insulin but not enough, which means glucose stays in the blood.

But now, a team of scientists in Sweden and Finland have identified five clusters of patients with diabetes. According to the researchers, each comes with significantly different characteristics and risk of complications.

Six different measurements were used across four separate studies: age at diagnosis, body mass index (BMI), long-term glycemic (blood sugar) control, the function of insulin-producing cells in the pancreas, insulin resistance, and the presence of specific auto-antibodies linked to autoimmune diabetes.

Instead of splitting diabetes simply into type 1 and type 2, the researchers came up with five different disease profiles – one autoimmune type of diabetes and four other distinct subtypes.  All five types were found to be genetically distinct, with no shared mutations.

According to researchers, this is enough to suggest that there are five distinct diseases that all affect the same body system, rather than the same disease at different stages of progression.

 

A recent blockbuster research study revealed a more precise breakdown of the types of diabetes, suggesting that instead of the recognized Types 1 and 2, there are five subsets of the disease.  The new finding is likely to help doctors target treatments that are more personalized.

Diabetes No More

The Scandinavian researchers found that their diabetic subjects fell into five distinct clusters:

  • Cluster 1. Severe autoimmune diabetes. This group is like the classic Type 1.
  • Cluster 2. Severe insulin-deficient diabetes. Like Cluster 1 but the immune system wasn't the cause of their insulin deficiently. This group has the highest risk of retinopathy, a complication of diabetes that can cause vision loss.
  • Cluster 3. Severe insulin-resistant diabetes. This occurs when people are overweight and highly insulin resistant which means that their bodies produce insulin, but their cells don't respond to it. This group has the highest risk of kidney disease.
  • Cluster 4. Mild obesity-related diabetes. People have a milder form of the disease without as many metabolic problems and they tend to be obese.
  • Cluster 5. Mild age-related diabetes. This form is like cluster 4 but people are older when they are diagnosed. This cluster is the most common form, affecting about 40 percent of the people in the study.

Many doctors treat their patients as individuals as they tailor treatment to the patient, establishing who needs oral medications and who needs insulin. They also prescribe diet and exercise for all. A big part of my task is to figure out what is keeping someone from succeeding at gaining control of their blood sugar.

The researchers do note some limitations though: there's no evidence yet that these five types of diabetes have different causes, and the sample only included Scandinavian patients, so a wider study is going to be required to investigate this further.

Millions more are thought to have pre-diabetes, or blood sugar levels that are above the normal range but not yet high enough to be classified as diabetic.
Common warning signs include:

  • feeling very thirsty
  • peeing more regularly, especially at night
  • feeling really tired
  • weight loss and loss of muscle bulk
  • itching around the penis or vagina, frequent bouts of thrush
  • cuts or wounds than take ages to heal
  • blurry vision

Type 1 diabetes, which is a serious lifelong condition that typically requires insulin injections or pump, can develop quickly over a matter of weeks, even days.
In contrast, people can live with type 2 diabetes for years without realizing it, but the condition can be reversed by adopting a healthier lifestyle and losing weight.

Satchidananda Panda, a professor at Salk Institute for Biological Studies in San Diego, first tracked the effects of time-restricted feeding in mice. In 2015, he expanded his studies to include humans.

The findings, published in the journal Cell Metabolism, showed that when eight overweight people who naturally ate for 15-plus hours a day restricted their eating to a 10-hour window for 16 weeks, they lost 4% of their weight. A year later, they reported sticking to the plan, even though they didn’t have to, and had kept the weight off.

In TRF diet, you can eat whatever you want and as much as you want—just not whenever you want. Daily food intake should be limited to a 12-hour window, and ideally cut down to eight to 10 hours. But you can pick the hours you want to eat.

Despite a lack of dietary restrictions, people following TRF diet end up consuming fewer calories and lose weight, according to studies and experts. Preliminary evidence also shows other health benefits of fasting for 12 hours or more, including lower blood pressure and improved glucose levels, and physiological changes linked to slowing the aging process.

Dr. Panda is working on a new, larger clinical TRF trial of 150 firefighters. Funded by the U.S. Department of Homeland Security, the study is a collaboration with Pam Taub, a cardiologist and associate professor of medicine at the University of California San Diego Health System. Dr. Taub is also conducting a separate study in which UCSD patients with metabolic syndrome restrict eating to 10 hours a day. Preliminary data show weight loss and improved fasting glucose levels.

TRF diet is basically form of intermittent fasting. TRF diet focuses on when to eat rather than what to eat. Limiting the hours, you eat each day may help you consume fewer calories. It may also provide health benefits, including weight loss and improved heart health and blood sugar levels.

An example of time-restricted eating is if you choose to eat all your food for the day in an 8-hour period, such as from 10 a.m. to 6 p.m. The remaining 16 hours each day are the fasting period, during which no calories are consumed. This same schedule would be repeated every day.

Most people eat from the time they wake up until the time they go to bed. Switching to TRF diet may cause you to eat less. However, it is possible some people may eat more in short duration of TRF diet. However, one study found that when healthy adult men limited their eating to about a 10-hour window, it reduced the number of calories they ate each day by about 20%.

You are not alone. Many people have turned to fish oil or vitamin B12, so they are victim of dementia. But solution to keep your memory alive may be as simple as eating salad every day.

Nutritional epidemiologist Martha Clare Morris and her team at Rush University Medical Center in Chicago found that people who ate one to two servings of leafy green vegetables each day experienced fewer memory problems and cognitive decline compared to people who rarely ate spinach. She states that veggie lovers who included about 1.3 servings a day into their diets had brains that were roughly 11 years younger compared to those who consumed the least amount of foods like spinach or kale.

The study included 960 people, all between 58 and 99 years old and all without dementia. Everyone enrolled in the study was part of the Memory and Aging Project, which has been ongoing since 1979 at the Knight Alzheimer's Disease Research Center at Washington University. Scientists followed up with participants for 10 years and discovered that the rate of decline for those who ate the most was the equivalent to being 11 years younger in terms of brain age. The study was published Wednesday in the journal Neurology.

According to Morris, it’s not just leafy greens that could potentially keep our memories alive, but the nutrients found in these vegetables that may be responsible for the brain boosting benefits.

As good as it sounds, it is to be noted that this study was observational, thus the data provide no concrete evidence of a causal relationship between spinach and brain health.
But you can’t go wrong by adding salad your daily eating routine. It may just help keep your memories activated while you are alive.
